that functions in this role must be able to effectively maintain
required records and documentation. The primary role of this
position is to provide Intensive In-Home Services, an enhanced
mental health service. Intensive In-Home Services is a team
approach designed to address the identified needs of children and
adolescents, who due to serious and chronic symptoms of an
emotional, behavioral, and/or substance use disorders, are unable
to remain stable in the community without intensive interventions.
This is a time-limited, intensive child and family intervention
based on the clinical needs of the youth.As a service provider, the
Qualified Professional, under the direction of the team leader,
assists the person receiving services with coping and 1) to diffuse
the current crisis, evaluate its nature and intervene to reduce the
likelihood of a recurrence; 2) ensure linkage to needed community
services and resources;(assisting the team leader in consulting
with identified providers, engaging community and natural
supports,) 3) provide self-help and living skills training; 4)
provide parenting and independent living skills to help the
consumer and family build skills for coping with the identified
disorder; 5) monitor and manage the presenting psychiatric and
addiction symptoms; 6) work with caregivers in the implementation
of home-based behavioral supports. The QP, in addition to
participating in treatment team planning and assistance with PCP
development, works with the person receiving services and his/her
family to follow-up on progress/lack of progress in meeting
treatment goals. The QP assists the clinical team with arranging
and providing psycho-education as indicated in the Person Centered
Plan; assists the team leader in monitoring and evaluating the
effectiveness of interventions, as evidenced by symptom reduction
and progress toward goals identified in PCP, this includes
telephone time and collateral contacts to persons who assist the
recipient in meeting his/her goals specified in the Person-Centered
Plan.The QP assists and supports the person served in developing
critical daily living and coping skills. Other responsibilities
include, but are not limited to, advocating on the behalf of the
person receiving services, documentation of daily progress notes,
implementing the interventions identified in the PCP and other
duties as assigned. The Qualified Professional has the ability to
deliver services in various environments, such as residential,
school, workplace, and community settings. These interventions are
strength-based and focused on promoting recovery, symptom
reduction, increased coping skills, and achievement of the highest
level of stabilization within the family's home, and other goals as
developed according to the Person-Centered Plan (PCP).The QP serves
on the Intensive In-home Team comprised of one full-time team
leader and at least two additional full-time equivalent positions
as follows: one FTE team leader who is a Licensed Professional who
has knowledge, skills, and abilities required by the population and
age to be served (may be filled by no more than two individuals),
one FTE Qualified Professional who has knowledge, skills and
abilities required by the population and age to be served (may be
filled by no more than two individuals), and one FTE Qualified
Professional or Associate Professional who has knowledge, skills
and abilities required by the population and age to be served (may
be filled by no more than two individuals). The team to family
ratio will not exceed 1:12 for each three person team.The QP
actively participates in clinical supervision provided by the team
leader in accordance with his/her individualized supervision plan
and contract according to supervision requirements.JOB
SPECIFICATIONSQUALIFICATIONSEducational and Job
Requirements:Bachelors degree in Human Service field with 2 years
full-time, post-bachelors degree accumulated MH/DD/SAS experience
with population, ORBachelors degree in field other than Human
Services with 4 years full-time, post bachelors degree accumulated
MH/DD/SAS experience with population, ORAn individual who holds a
license of Registered Nurse (RN) in North Carolina, and also has 4
years experience with MH/DD/SAS population. The QP function may be
completed by a Certified Clinical Addiction Specialist (CCAS) or
Certified Clinical SupervisorORMaster's degree in Human Services
field and has 1 year of full -time post-graduate degree accumulated
MH/DD/SAS experience with population.CERTIFICATES, LICENSES,
REGISTRATIONS:Valid drivers license (this is a condition of
employment) including personal vehicle insurance coverage. Must
maintain proficiency in company sponsored training and
certifications and privileging if required.Work Environment: Work
is performed primarily in the client's living environment and in
the community and may involve exposure to cigarette smoke, domestic
animals/pets, household pests, uncomfortable heating/cooling, and
other issues related to the domestic location in which service is
delivered.
