Job Description:
Job Description Coastal Horizons Center, Inc. has an opportunity
for a Co-Responder Care Navigator to become part of our
Co-Responder Pilot Program. This is a collaborative program with
New Hanover Division of Social Services (DSS) to jointly respond to
members experiencing mental health, substance use, and/or I/DD
needs. This role provides clinically necessary tasks to stabilize
individuals and/or families through a collaborative role with the
licensed therapist and DSS staff. The Co-Responder Care Navigator
also: Will improve positive client outcomes through linking clients
to medication management, coordinating care, educating clients,
positive role modeling, building trust between clients and their
clinical and medical practitioners, supporting and connecting them
to community programs, and enhancing communication and the
continuity of care. Interact with all members of the healthcare and
DSS team to keep the lines of communication open. Teach, mentor and
monitor clients on issues relevant to their care. Respond to
clients at the initial point of crisis or in subsequent follow up
appointments over a duration to ensure stability of the member.
This position will work largely in the client---s home and/or
community settings. This position is intended to keep a flexible
schedule that could include nights and weekends, and afterhours
crisis response. Qualification/ Skills: Education: Bachelor---s
Degree in the Human Resources field required. Meets requirements
under 10A NCAC 27G.0104 as a Qualified Professional for Mental
Health Experience/ Skills: Two (2) years of experience in the field
required. Preferably experience in community crisis response.
Trained to provide evidence-based care coordination, brief
behavioral interventions, clinical screeners, Social Determinants
of Health screeners, and to support the treatment recommendation
follow through. Flexible schedule that could include nights and
weekends, and after hours crisis response. Salary: Commensurate
