year of Hospice Experience License: Must have active nursing
license Certifications: CPR, BLS Must-Have: Recent specialty
experience Description: Responsible for coordinating and directing
home nursing care according to physician's orders. Care is provided
based on established policies and procedures. Utilizes the nursing
process in a competent manner. Reports directly to the Clinical
Manager. Supervises and evaluates Home Health Aides and Licensed
Practical Nurses/Licensed Vocational Nurses. Requires home visits
and periodic office meetings. Must have reliable transportation, a
current driver's license, and appropriate liability insurance.
Average caseload consists of 5-8 patients per day. The role
includes various types of visits such as Start of Care, Wound Care,
and Follow-up Visits. All RNs must be proficient with HomeCare
HomeBase and Oasis. Must travel up to 75 miles from the home clinic
for visits. The commuting time to and from the first and last
patient is not billable. Local applicants are accepted. Recent and
relevant experience in specialty required. Will participate in
on-call rotation shared with case managers. Fulfills staffing needs
due to recent resignations. Engagement in weekend rotation may be
required based on the location and profession. May be required to
