New Hanover County is seeking a Social Worker or Public Health Nurse to serve as a Hepatitis C Bridge Counselor at the health department. Grant funded through the state of North Carolina, this position provides care coordination services for persons infected with HCV, linking them to clinical providers for viral hepatitis care and treatment as well as referring providers to the Viral Hepatitis Program manager, the Hepatitis C Nurse Consultant, or to the NC Hepatitis Academic Mentorship Program (CHAMP) website. Duties include facilitating a rapid disease intervention by engaging patients and serving as the first point of contact; providing ongoing support and monitoring for each referral; collaborating with key community stakeholders to identify resources and coordinate the development of a regional resource list; conducting contact investigations for acute HCV; collecting data and complete quarterly reports; providing performance measures reports and other duties as assigned.

Hours:

Hours are 8:00 am - 5:00 pm Monday - Friday. Must be available to work during emergency situations.

Salary Information:

The current hiring range for this position is$56,351 - $76,074.Salary will be determined based on qualifications, internal equity, budget, and market considerations.

Minimum Qualifications for Social Worker Applicants:

* MSW from an accredited school;


* OR BSW from an accredited school of social work and 1 year of related social work experience;


* OR a four-year degree in a human services field with at least 15 semester hours in courses related to social work and 2 years of related social work experience;


* OR a four-year degree and 3 years of related social work experience.



Minimum Qualifications for Public Health Nurse Applicants:

* Valid RN license
* Bachelors Degree in Nursing
* OR Associate's Degree in Nursing


* OR graduation from an accredited school of professional nursing





1 year of professional nursing experience

Must be able to lift up to 25 pounds of equipment.

* Valid NC Driver's License
