Interdisciplinary Team (IDT), the Homestead Hospice and Palliative
Care social worker helps patients, families, and caregivers with
emotional needs and community resources related to their end of
life journey.Qualifications:Graduate of a school of social work
approved by the Council of Social Work Education with a master's
degreeSocial Work license preferred in accordance with each state's
specific regulationsMinimum of one (1) year social work experience
in a health care setting, preferably hospiceLicensure by the
stateValid and current LCSW or LMSW licenseExcellent listening and
communication skillsResponsibilities:Knowledgeable regarding
psychosocial and emotional factors that impact multiple health
issues; understanding of medical regimens, psychiatric diagnoses,
social work treatment interventions, and Community health/welfare
resourcesUnderstanding and acceptance of hospice principles and
philosophy of careAbility to establish and maintain a therapeutic
relationship with patients, families, and colleaguesAbility to
function as an active member of Homestead Hospice and Palliative
Care IDT to create/implement a plan of care and provide education
to IDT on social and emotional factors related to care of
terminally ill patients and their familiesKnowledge of the impact
of terminal diagnosis and anticipatory grief on psychosocial
functioning, family dynamics, and community relations, and of
appropriate therapeutic interventionsAbility to effectively provide
individual, group, and family counseling, crisis intervention, and
community referrals in accordance with physician's ordersAssesses
patient/family psychosocial response to terminal illness. Evaluates
coping skills in crisis situationsExplores and evaluates
patient/family financial situation and offers appropriate resources
as indicatedAssists in transfer or discharge planning from
Homestead Hospice and Palliative Care services when requiredAssists
in providing individual and/or group bereavement counseling for all
hospice families and staff, when appropriate, to the general
CommunityEducates and completes initial volunteer assessment for
patients, families, and caregivers; reassess as neededEducates and
completes initial bereavement assessment for patients, families,
and caregivers; reassess as neededProvides workshops and
presentations on topics related to grief, loss, and bereavement for
professional groups and Homestead staff as appropriateIncreases
public awareness of hospice and palliative services through
presentations to community groups as appropriateDocuments timely in
EMR on all patients including initial psychosocial assessments,
progress notes, and care plansAbility to provide appropriate tele
mental health interventions when appropriateAdheres to CMS, state
guidelines, social work standards of practice and Homestead Hospice
and Palliative Care policies and proceduresPerforms other duties as
assigned
