Job DescriptionPosition Purpose: Conducts home visits to perform a comprehensive health re-assessment regarding clinical, behavioral and home safety requirements on existing members in the community as required by the state for the Managed Long Term Care program. -- Identifies applicant's health needs and completes medical assessment of applicant. -- Tracks assessment schedule and updates assessments in a timely manner. -- Initiates direct communication with health care providers involved in care of the applicant to obtain complete and accurate information as needed. -- Collects information concerning ongoing eligibility. -- Coordinates with Care Management team and provides updates regarding member health status. -- Inputs visit and assessment data into systems -- Adheres to all HIPPA standards and confidentiality requirements. -- Performs other duties as assigned. Education/Experience: A Bachelor's Degree in Nursing or related field. 2+ years of experience in a clinical acute care setting, preferably in home health and/or geriatrics, Long term support services, Managed care, other Field work preferred For North Carolina only: * As a CORE RN you will provide primary telephonic care management to members who are stratified as High, Moderate, or Low. These members may require face to face interaction. * Support may be required to collaborate with the LTSS program. The goal of the LTSS Program is to support members to live in the lowest acuity setting that effectively meet their personal, clinical and safety needs; deliver expanded community services to support member needs beyond acute care. Care managers interact with members both telephonically and face-to-face, depending on the needs of the member and their level of care needs. * Care managers identify member needs through various standard assessments; care managers will also develop an individualized care plan for each member. You, as the CORE CM acts as an advocate and liaison for your member. You communicate with the member, their providers, their community services, and their support team to ensure that the member's needs are met. Benefits include those used by members of all ages with functional limitations and chronic illnesses. * Must reside in the state of North Carolina within regions as identified below * Must be willing to travel to office as needed for training, development and meetings once it is safe to do so * Must be willing to travel within your assigned region and neighboring region to support peer coverage as needed once it is safe to do so Region 1 - Bryson City, Brevard, Asheville, Newland, Lenoir, Rutherfordton, Burnsville, or Morganton OR Region 3 - Charlotte, Wadesboro, Statesville, Salisbury, Concord, Albermarle, Monroe, Gastonia, Lincolnton or Cleveland.
