Position Summary:RN Clinical Manager will be part of a multi- disciplinary management team and responsibilities include departmental operations and strategic planning. All new initiatives and services will utilize the lean methodology and will follow with standard work. The ability to integrate the bio psychosocial (medical, behavior health and social) approach into a comprehensive care planning for high risk participants. Main areas of responsibility include clinical care pathways, bundled payments, mobile outreach clinic, outreach events, and any other areas that are developed related to clinical focus and/or requiring nursing expertise. The RN Clinical Manager requires independent decision making skills to effectively manage the following responsibilitiesClinical Nursing Responsibilities:

Develop and evaluate models to capture high risk participants needing focused servicesExceptional core customer service skills for participant engagementThis position will manage a case load of high risk individuals and provide supervision to the clinical RN Navigators working under themThis position will provide management oversight as needed for all nurses in CHR that are managed/supervised by non-nursing manager/supervisors.Oversees documentation compliance ensuring metric measurements are maintained in accordance to CMS and grant requirements and provide analysis of clinical impact.Care will be provided in various locations including in the participants home, group settings and one to one educationAn ability to take the information from all assessments to establish a plan of careDeveloping and managing wrap around supports necessary to manage the participant's health care needs and access examples include group education, partnership with YMCA and Health Point, Diabetes Presentation Program, home care and personal care providers.Build partnerships with extended care providers to ensure successful transitions from acute to post-acute settingsOversight of support groups and one to one education using best evidence based pathwaysParticipation in community coalitions and committeesWill be required to transport participants to healthcare appointments, as needed, utilizing TCCN departmental vehicle Budget Responsibilities:
Ensures compliance with fiscal management of resources allocated to contracted services Accountability to Department budget to include staff mileage, training and supply expensesManagement of overtimeBuilding partnership to ensure shared risk of uncovered expenses Reporting Responsibilities:
Transitional Care Matrices Use data to drive planning Dashboardsanalysis of metrics, planning of countermeasures and monitoring of outcomesAssist with the completion and routine monitoring of the Community Benefit report; including support for ongoing data collection and ensuring identified needs are considered in the development of the strategic direction for the organizations wellness initiatives. Supervision Responsibilities:
Ensuring direct reports adhere to departmental and organizations policies and proceduresResponsible for hiring, orientation, ongoing training to ensure staffs understanding of responsibilities. Facilitating group and individual meetings as needed
  • Ensuring Policy and Procedures are developed and reviewed annually for clinical navigation and other policies as identified.Ensure documentation compliance though internal audits.Annual performance appraisalsAnnual CompetenciesPayroll Other Responsibilities Include:

  • Ensure department participation in hospital sponsored community screeningsQUALIFICATIONSExperience:
    Minimum of three years hospital based nursing experience required. (Not for profit community hospital/system experience preferred)Minimum of three years previous experience in community health education, public health or related experience required.Minimum of 2 years general supervisory/manager experience including human resource management preferred.Experience in development and implementation of care strategies to improve population health preferred. Education: Bachelor of Science in Nursing Required. License/Certification:
    Current SC RN licensure required (or equivalent compact state SC eligible license)Certified Diabetes Educator preferred.American Heart Association Healthcare provider course with Basic Life Support (BLS) certification required to be initially completed with the GHS Staff Development Office within 30 calendar days of hire date (exceptions to obtain certification from a provider other than the GHS Staff Development Office are approved by Staff development on a case by case basis). BLS Recertification is required every two years. Knowledge/Skills:
    Ability to work independently, adjust priorities often and establish rapport quickly Demonstrated experience in diplomacy and ability to contribute to team goal setting and program development Proficient PC skills and experience of Windows and Microsoft office programs required.Strong interpersonal and communication skills. Other:
    Personal transportation required to conduct services, home visits, and transport materials and supplies to service location throughout the region.Valid SC driver's license in good standing Physical Requirements: Light-Medium Physical Agility Test Rating Required (PAT)While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; frequently to use hands, fingers; and frequently to talk or hear. The employee must exert 15 to 20 pounds of force occasionally (activity or condition exists up to 1/3 of the time), and/or 5 to 10 pounds of force frequently, and/or greater than negligible up to 5 pounds of force constantly to move objects. The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job and will be tested by a post offer PAT.
